We are offering block bookings or ad-hoc shifts within the Bristol area. Rates of pay varying from £20.00 – £30.00 per hour, we also offer flexibility in the way you are paid.

THE ROLE:

Community nursing teams provide a wide range of care and support to housebound and temporarily housebound individuals who are over 18. This includes:

  • Coordinating care for individuals with long term conditions
  • Administering medication and supporting self-administration
  • Dressing and managing wounds
  • Providing Catheter Care in Clinics and within the individuals home
  • Providing Wound Care for housebound individuals after surgery
  • Supporting issues around continence
  • Caring at end of life

  • A current NMC Registration and have a minimum of six months of qualified general nursing experience.

NHS Community Nurse

THE ROLE:

The post holder will work as part of the District Nursing Service within the integrated Multidisciplinary team under the supervision of the District Nurse / Caseload holder. He / She will be accountable for the delivery of care to an identified caseload, to patients within their own home at GP premises/health centres or other community setting. He / She will manage the caseload in the absence of the District Nurse/ Caseload holder.

  • Work in accordance with NMC Code of Professional Conduct and scope of Professional Practice.
  • Provide high quality skilled nursing services to patients within their own home at GP premises/health centre’s or other community setting using evidence based practice in liaison with other members of the team.
  • In conjunction with the District Nurse/ Caseload holder, take responsibility for the assessment, planning, implementation and evaluation of allocated patients needs.
  • Identify risks through the assessment process documenting and initiating appropriate action, reporting as appropriate to the District Nurse/ Caseload holder.
  • To develop and maintain evidenced based practice in health needs, specifically tissue viability, long term conditions management and palliative care.
  • Provide support and advice to patients and informal carers, including the teaching of basic nursing care as appropriate.
  • Participate in the promotion of healthy living and the prevention of ill health and disability through diagnostic screening, education and promotion of independence and self management as a priority of care in the community.
